Brochure Order Request

Our organization provides free of charge safe sleep brochures and postcards to other organization in the state of Illinois. If you are an organization or hospital and would like to order educational print materials, please click the download button.

There are 3 ways you can submit your Order Form:

1. Email Lucy@sidsillinois.org a completed copy of your form.

2. Fax our organization at 630-541-8246

3. Mail in your request to SIDS of IL, Inc. 6010 Route 53, Suite A, Lisle, IL 60532

Please allow 1-2 weeks for delivery. SIDS of IL, Inc. pays for the shipping and printing of these items. There is a quantity limit of each item to ensure there is enough product for the entire state of Illinois. Our printing costs are 100% paid by donations throughout the year. We appreciate all who work with us and share our various fundraising pages to help ensure we continue to cover the cost of these services. Thank you!
