Crib Referral - Project Safe Sleep

Do you have a client that is in need of a safe sleep place for their baby? We offer Cribettes through our Project Safe Sleep Program. If your client is eligible for the program, they will receive a call from one of our qualified educators and upon completion of our safe sleep education, they will receive a Cribette. Please see if your client qualifies below.

WE DO NOT ACCEPT SELF REFERRALS. PLEASE DO NOT HAVE YOUR CLIENTS CALL OUR OFFICE.

**Attention BCBS Medicaid Insurance Clients: If you (or your client) has BCBS of Illinois Medicaid as their insurance provider, you are qualified for a Cribette through our "Special Beginnings Program". Please call the number on the back of the insurance card, ask to speak to a "care coordinator" and tell them you are in need of a crib.
